Talk to Doctor Henry at the Jacobstown Lodge
You can unlock the Guess Who I Saw Today Side Quest by talking to Doctor Henry in his clinic, which is located inside the Jacobstown Lodge on the northeastern side of the Mojave Wasteland.

Talk to Doctor Henry at the Jacobstown Lodge
Upon arriving at Jacobstown, head towards the lodge, the large building surrounded by mutants. Once inside, you can find Doctor Henry's clinic on the right side, past the stairs.
Recruit Lily Near the Lodge Entrance (Optional)
After talking to Doctor Henry inside the Jacobstown Lodge, you can head outside and talk to Lily in front of the Lodge to recruit her. This is completely optional and you can progress without Lily to complete this Side Quest.
Explore the Charleston Cave
Make your way to the hidden cave on top of the hill behind Jacobstown. Here, you will encounter a couple of Nightstalkers as you progress further in the cave.
Continue moving forward until you reach the Nightkin corpse that contains the Chewed Stealth Boy. Take this item, and then leave the cave before fast traveling back to Jacobstown.
You Can Also Grab the Oh Baby Melee Weapon
In the same room where you find the Nightkin Corpse, you can also find a unique melee weapon lying on the floor called Oh, Baby.
Return to Doctor Henry With the Stealth Boy
Return to Doctor Henry at the Jacobstown Lodge and give him the Stealth Boy that you obtained from the cave.
Request Lily to Help With the Experiment
After talking to Doctor Henry, you need to speak with Lily and request for her assistance with the experiments. The experiment will immediately commence, and you must wait for Doctor Henry to finish his test before being able to progress further.
Speak With Keene When He Enters the Room
After the experiment is over, Keene will enter the room and demand that you hand over the Stealth Boy. At this point, you will have three options to choose from on how to resolve the conflict.
Refuse to Give the Prototype
If you refuse to give Keene the prototype, he and his friends will become hostile and immediately attack you.
Give the Prototype to Keene
Letting Keene get the prototype will also defuse the situation but Doctor Henry is going to be worried of what the mutants will do next.
Convince Keene Stand Down
You can also defuse the situation by convincing Keene that his actions affect all mutants in the wasteland. This option is only available if you have 80 Skill Points in Speech and you will be rewarded with an additional 80 XP.
Talk to Doctor Hentry and Discuss What to Do
Continue your conversation with Doctor Henry and decide whether Lily should continue wearing the prototype despite her near-death experience, or if Lily should remove it and stop the experiment altogether.
Let Doctor Hentry to Continue the Tests on Lily
Telling Doctor Henry to continue his tests on Lily will end the side quest and reward you with 500 XP.
Tell Doctor Henry to Stop the Experiments
Asking Doctor Henry to stop the experiments will ensure Lily's safety and conclude the side quest, also granting you 500 XP.
Convince Hentry to Test on Nightstalkers Instead
If you have 90 Skill Points in Science, you can persuade Doctor Henry to continue his tests on the Nightstalkers instead. This will keep Lily safe, allow Doctor Henry to continue his experiments, and end the side quest, rewarding you with 590 XP.
Best Choice for Guess Who I Saw Today
Avoid Conflicts with Keene and Convince Doctor Henry
When Keene arrives at the clinic to take the prototype, defuse the situation by convincing him to leave. This will require 80 Skill Points in Speech. This way, you avoid killing Keene, and Doctor Henry can keep the prototype.
As for Doctor Henry's dialogue options, it is recommended that you convince him to continue his tests on the Nightstalkers instead. This requires 90 Skill Points in Science. This outcome provides the best possible reward, giving you 590 XP for the Side Quest.
